Background: Adenoid cystic carcinoma (ACC) is a relatively common malignant tumor arising from minor or major salivary glands. This tumor extends through perineural invasion.Case Report: We present a 58 year- old female with limited movement of right eye and non-pulsative vertex headache. Physical examinations showed involvement of several cranial nerves, indicated intracranial extensions. In sinus endoscopy, a mass was seen in right middle meatus. Conclusions: Planned combinations of surgery with radiotherapy have shown improvement in both local control and survival.
