The 12-lead ECG showed sinus tachycardia, normal axis, left ventricular hypertrophy, and widespread horizontal ST-segment depression in I, aVL, II, III, aVF, and V4–V6, and ST elevation in aVR and V1. In the setting of acute coronary syndrome, this ECG usually indicates left main heart disease or a significant proximal LAD occlusion.
