Note the clear fall in systemic pressure when crossing the pulmonary valve and subsequent pressure recovery after catheter pullback. This can be explained by mechanical obstruction of a narrowed pulmonary orifice and relief of this obstruction after catheter withdrawal. This is similar to Carabello’s sign in critical aortic stenosis.
