Figure 2. Schematic diagram of the surgery using CLAP. Two iMAP pins are inserted into the cortical bone pseudocavity, the first proximally and the second distally. Following minimal curettage through the cortical window, multiple drill holes are created from the cortical surface, through the pseudocavity, and into the medullary cavity using a 0.8 mm Kirschner wire. Additional drilling with a 1.0 mm Kirschner wire was performed to connect isolated pseudocavities separated by septa. An iSAP tube was positioned in the soft tissue near the cortical window. CLAP – continuous local antibiotic perfusion; iMAP – intramedullary antibiotic perfusion; iSAP – intra-soft tissue antibiotic perfusion; K-wire – Kirschner wire.
