Small bowel intussusception due to multiple lipomas of the jejunum. A report of an uncommon case and a literature review

Abstract

Background: Although small bowel neoplasms are not relatively often, lipomas account for the third most
common benign lesions. They are usually solitary. The part of the small intestine in which
they are located the most rarely is the jejunum. Most of them remain asymptomatic. Possible
clinical presentations involve an intestinal intussusception, which is a relatively seldom condition in adults. Preoperative diagnosis is difficult. An intussusception is manifested as an acute
abdomen syndrome, thus a surgery remains the treatment of choice.Case Report: A young adult was admitted and operated on due to clinical symptoms of the acute abdomen. During laparotomy an intussusception of the proximal jejunum was found and surgical resection was performed. In resected specimen multiple submucosal tumours were identified. They were defined by the histopathologist as five lipomas located closely one to the other. The patient’s post-operative course was uncomplicated. No other lipomas at the gastrointestinal tract were noticed during peri-operative visual and manual examination and post-operativegastroduodenoscopy and colonoscopy, too.Conclusions: To sum up, the presence of clinical symptoms, the uncommon site at the jejunum, the multiple character of lipomas and the unusual patient’s age define an extreme rarity of the described case.
