Coexistence of coeliac disease and Leśniowski-Crohn’s disease

Abstract
Summary
Background: Nowadays more and more attention is paid to latent coeliac disease, which can be observed in adults, and can cause a lot of diagnostic problems. Coexistence of coeliac disease (gluten enteropathy) and other autoimmune diseases is better and better proved.
Case report: We report a case of 27 year old male suffering from gluten enteropathy coexisting with LeÊniowski- Crohn’s disease. Total remission of symptoms of both diseases was achieved after treatment with glicocorticoids, mesalazine, Metronidazole, and gluten-free diet.
The coexistence of gluten enteropathy and autoimmune diseases is well documented. However there are very few reports on coexistence of gluten enteropathy and inflammatory bowel disease (IBD). The similarity of symptoms of gluten enteropathy and IBD may be misleading. It makes differentiation difficult and delays proper treatment, quite different for each disease. In presented case the diagnosis of LeÊniowski-Crohn’s disease was confirmed by typical endoscopic and microscopic picture of gut mucosa, perianal abscess and good results of treatment. The basis of gluten enteropathy diagnosis was the presence of antigliadin, antiendomysial, anti-tissue
transglutaminase IgA antibodies.
Conclusion: In differential diagnosis of autoimmunologic diseases gluten enteropathy should be taken into consideration.
