Background: Leptospirosis may develop after direct exposure to urine of infected animals or urine-contaminated water and soil, through occupational activities.Case Report: Herein we report a case infected with leptospira after repairing the sewer system of house. A 34-year-old previously healthy male presented to the emergency department with icter, renal failure and elevated tramsaminase levels. Penicillin was given for ten days, and the patients discharged without complication. Conclusions: Occupational activities related to contaminated water predispose to leptospirosis and a patient who present with icter and acute renal failure the need for a high index of suspicion of leptospirosis.
