Usefulness of ultrasound biomicroscopy in selecting the site and energy level of impact in laser surgery

Abstract

Background: The aim of the paper is to analyze the usefulness of ultrasound biomicroscopy (UBM) examination technique in selecting the site and impact energy level in eyes with non-transparent optic centers, reduced sclera thickness and anatomical structures of the anterior eye segment affected by trauma.
Case report: A 31-year-old patient with traumatic glaucoma, 29 mm anatomical length of the eyeball, keratectasia with leucoma, and multiple scleral staphylomas extending to the corneal limbus, was qualified for contact transscleral cyclophotocoagulation (TSCPC). The patient’s intraocular pressure had not normalized despite filtration procedures repeated three times and perforation diathermocoagulation, and the eye remained painful. Ultrasound biomicroscopy was performed in order to select the optimal site for TSCPC.
Conclusions: In eyes with altered anatomical relations of the anterior eyeball segment, UBM allows to select an appropriate site for TSCPC. Ultrasound biomicroscopy can also facilitate the adjustment of impact energy level to thickness of the sclera.
