Intramuscular injection of co-formulated organophosphate and synthetic pyrethroid: a case report

Abstract
Background: Organophosphate insecticides and synthetic pyrethroid insecticides are widely used for the purpose of ectoparasite control in sheep they are regarded as veterinary medicines. They are cholinesterase inhibitors.
Case Report: An 18-year-old woman was poisoned by an intramuscular injection of 2 mL of a propetamphos and cypermethrin co-formulation (consisting of propetamphos 2.7% and cypermetrin 5.5% in water). Clinical fi ndings were somnolence, hypotension, bradycardia, and edema and hyperemia at the injection site.
Conclusions: Clinical signs of intramuscular organophosphate and synthetic pyrethroid intoxication are similar with oral, dermal and inhalation exposure. However early signs are seen with low doses with intramuscular exposure. This poisoning is a serious condition that needs prompt diagnosis and treatment. Intoxication with higher doses occurring from intramuscular injection may result in more serious problems.
