Solitary metastasis from occult thyroid cancer mimicking a convexity meningioma

Background: Intracranial metastatic thyroid cancer is rare. We report the case of an 82-year-old woman with a solitary brain metastasis from thyroid follicular carcinoma that mimicked a convexity meningioma.
Case Report: The patient was admitted to our hospital for appearance of generalized seizure. Magnetic resonance imaging showed a single mass in the frontal region. Further radiological examinations revealed no additional lesions. After making a provisional diagnosis of meningioma, we performed radical surgery. Histopathological evaluation of the resected tissue was consistent with metastasis of thyroid follicular carcinoma. The patient underwent total thyroidectomy and histopathology confirmed a follicular subtype of silent carcinoma. Postoperative follow-up showed no disease progression for 4 years. After that, recurrence occurred in the same anatomical site and was fatal.
Conclusions: This is the first report that describes a long-lasting remission after surgical excision of an isolated meningeal metastasis from occult thyroid follicular carcinoma in an elderly patient. Our experience shows that if conditions allow, surgery alone can help prolong survival and improve quality of life in patients of advanced age.
