An unusual cause of epididymo-orchitis and a new therapy regimen: brucellosis

Abstract

Summary
Background: Brucellosis is a zoonotic infection transmitted from animals to humans especially by ingestion of infected food products. Various complications may develop during the course of brucellosis.
Brucellosis is one of the rare causes of the epididymo-orchitis. The best regimen for the treatment of brucellosis has not been clearly determined. Aimed at presenting these case reports in purpose of discussing epididymo-orchitis cases due to Brucella infection and new
information in the literature on the treatment of brucellosis.
Case reports: This manuscript describes two cases of epididiymo-orchitis due to Brucellosis. The diagnosis in both cases was based on clinical presentation, epidemiologic setting and serologic testing. A
treatment strategy consisting of 3 antimicrobial agents is proposed: 45 days of Rifampin/Doxycycline combined with Streptomycin for the first 10 days of treatment. Complete resolution was achieved with medical treatment and testicular masses diaappeared in all patients. Six months after therapy they felt well. A relapse did not occur.
Conclusions: Brucellosis induced epididymo-orchitis is a recognized clinical problem in endemic regions, requiring early detection and appropriate medication. Triple antibiotic regimen (doxycycline,
streptomycin and rifampicin) for the treatment of brucellosis should be study and evaluate in more patients.
